I hereby authorize One Planet Youth Fair to publish photographs and videos taken of me and/or the below listed child, and our names, for use in the One Planet Youth Fair printed publications, website, and media outreach purposes.

I hereby authorized One Planet Youth Fair to publish videos submitted by my child when entering into the fair, for use in the One Planet Youth Fair printed publications, website, and media outreach purposes.

I attest that I am the parent, legal guardian that I have the authority to authorize One Planet Youth Fair to use below listed child’s photographs, videos and name.

I acknowledge that since participation in One Planet Youth Fair is voluntary, neither my child nor I will receive financial compensation for released media.
